یه آقای حدودا 60 سله که با درد شکم اومده بود درمانگاه ، درد RUQ داشت با یه توده بزرگ تو اپی گاستر با یه کوچکترم رو لبه کبد تو معاینه

بی اشتهایی و کاهش وزن و اینام داشت، قابل جراحی نبود فرستادیمش درمانگاه خون و درمان پلیتیو

A near 60 years old man, came to as a out patient, with abdominal pain in RUQ and a giant mass in epigastria and a modest one in liver subcostal edge in physical exam
The patient goes through loss of appetite and weight loss also
There was no surgical option, so we referred him to oncology clinic for palliative therapy

Acute Abdomen + Hepatosplenomegaly

یه آقای حدودا 30 سله که شب قبل با درد شکم اومده بود اورژانس، درد RUQ داشت که ریباندم داشت، تو سونو مایع آزاد داشت با هپاتواسپلنومگالی، همه آزمایشاش نرمال بود، گرافیشم خوب بود، هوای آزاد نداشت

توی سی تی، یه توده نزدیک کلیه داشت که لبولیشن چربی داشت (لیپوم مثلا) کبد وطحالش بزرگ بود، دور طحالش مایع آزاد داره، بافتشم ملتهبه، یه نقطه شبیه خونریزیم وسط طحالش داره، سابقه ضربه و ترومای شکمم نداشت، کلا بیشترین چیزی که بهش میخوره لنفومه

یه مشاوره خون براش دادیم ولی مریض رضایت داد و رفت

A near 30 years old man, came to ER last night, with acute abdomen, RUQ pain and tenderness + rebound tenderness In US, a hepatosplenomegaly was reported with mild free fluid All the lab data was normal, the CXR was normal too with no evidence of free air In CT, there was a mass near kidney with fat lobulation (e.g. lipoma) the liver and spleen size was larger than normal, and there was a fluid around the spleen and inflammatory tissue also A bleeding like spot was in the middle of spleen but the history of trauma was negative, the most likely diagnosis was lymphoma We asked for a hematology consultation, but the patients left the hospital
